Application for Current and Prospective Volunteers
Volunteer Application and Background Check
In order to ensure our students’ safety, prior to certain types of volunteering, school volunteers must complete a DPS Volunteer Application/Background Check form. This application is required if you plan to:
- Volunteer in the classroom more than five times per year
- Chaperone a field trip
- Lead a before or after school enrichment
You can apply online here. School volunteer background checks are good for one year. If you have completed the application in the past (not online), you must fill out a new form. No exceptions. This service is free.
After completing the online registration, bring your photo ID and fill out a Volunteer Confidentiality Agreement/Acknowledgment form (this can be done on the first day of volunteering). This form can only be completed in the Polaris Office.
Please call the Polaris office at 720-424-7860 if you have questions about the volunteer application.
